The Terms of Reference for the independent investigation into the death of a Clare teenager at University Hospital Limerick in 2022, have been confirmed.
16-year-old Aoife Johnston passed away from bacterial meningitis on December 19th 2022 after she was left on a trolley for twelve hours at UHL’s Emergency Department.
In late December 2023 the CEO of the HSE received a report (Systems Analysis) following the teen’s death..
At the time it was confirmed that there would be an independent and further investigation led by retired Chief Justice Frank Clarke. The HSE has this evening published the Terms of Reference.
Commenting on the process HSE CEO Bernard Gloster said: “The scope of the Independent Investigation is to provide an evidence-based report on the circumstances surrounding the death of Aoife and the clinical and corporate governance of University Hospital Limerick which led to the conclusions set out in the previous systems analysis report. The Judge has been asked to make any recommendations as he sees fit and to report directly to me.”
